Home Health Care encompasses a wide range of health care services provided in the patient’s home with the purpose of maintaining his or her maximal level of function, health, and comfort. Home care is a collaborative effort involving one’s family, physician, and an interdisciplinary home care team. It is a cost-effective alternative to extended hospitalization, rehabilitation, or a nursing home stay. Patients are usually more comfortable in their own home and studies have shown patients recover quicker at home.
The term “HOMEBOUND” means, “a condition due to illness or injury which restricts a patient’s ability to leave home without assistance.” Absences from the home are infrequent and of short duration.
After you have been discharged from the hospital or after a physician order, our trained staff will come to your home and assess your specific needs. This assessment helps us create your individual plan of care.
Patients must require intermittent skilled care, physical therapy or speech therapy. Patient must meet “homebound” criteria defined by Medicare guidelines and the patient must currently be under a physician’s care. All home health services must be ordered by a physician.
If eligible, Medicare covers 100% of your home health services. We coordinate your Medicare insurance coverage and bill Medicare on your behalf. We also accept private insurance, depending on your plan.
